Providence Clinics
Providence has 28 clinics that provide services in the areas of family medicine, immediate care, obstetrics/gynecology, internal medicine, dermatology, surgical services, pediatrics, and geriatrics. All clinics follow a universal financial assistance policy and are able to provide interpreter services. At the first visit most clinics require an initial deposit of $75 or more. At some clinics, uninsured patients may receive a discount or waive of this deposit.
General Financial Assistance Policy for Providence Clinics
- All uninsured patients receive a 25% discount on services.
- Patients must apply for OHP and be denied coverage in order to receive assistance from Providence. Please keep your denial letter from OHP to confirm your financial situation.
- After being denied by OHP, patients may apply for additional financial assistance by submitting a Financial Assistance Request Form (English Version, Spanish Version). Prospective patients are encouraged to apply for financial assistance prior to their clinic visit.
- Qualified applicants earning 200% or less of FPL will receive a 100% discount
- Required Information for financial assistance application: SSN, most recent tax returns, current bank statement, income verification, statement of support from family or friends providing financial assistance, copies of property tax assessments
