- Resident locations in zipcodes Multnomah County accepted.
- Age Restrictions: Between age 18 and 64.
Proof of Eligibility
- Picture ID
- Something with a printed address such as a bill
- Require proof of income which may include tax forms from previous year
Fees
- $15 per visit
- $10 for yearly pharmaceutical application services
