Eligibility Requirements
- FPL must be at or below 200% What is FPL?
- Resident locations in zipcodes Multnomah County accepted.
- Age Restrictions: Between age 18 and 64.
Proof of Eligibility
- Picture ID
- Something with a printed address such as a bill
- Require proof of income preferably tax forms from previous year accompanied by paycheck stubs or Oregon Trail cards.
Fees
- $15 per visit
- $10 for yearly pharmaceutical application services
