Eligibility Requirements

  • FPL must be at or below 200% What is FPL?
  • Resident locations in zipcodes Multnomah County accepted.
  • Age Restrictions: Between age 18 and 64.

Proof of Eligibility

  • Picture ID
  • Something with a printed address such as a bill
  • Require proof of income which may include tax forms from previous year

Fees

  • $15 per visit
  • $10 for yearly pharmaceutical application services

Interpretation Available for:

  • Must bring interpreter to each clinic visit
