  • Age Restrictions: Up to Age 100.

Proof of Eligibility

  • Same paperwork requirements as OHP

  • $20.00 Minimum Fee
  • Homeless - pharmacy fees may appy

  • Native Americans, Alaska Natives have priority (All Tribes and Bands)

Obtaining Appointments

  • Call 503.230.9875 to schedule an appointment
  • Walk-In Appointments Available? Walk in's may come in before noon - Monday through Friday
  • Screening Appointments: Done same day of appointment - arrive early to complete paperwork.
  • Wait Time To Be Seen
    • New patients within one week
    • Existing Patients depends on urgency - mostly same day/next day
