Coalition of Community Health Clinics Medication Formulary
Thanks to a generous donation from Providence Health. the Coalition through the Pharmacy Bridge Program has access to a Pharmacy Benefit Manager (PBM).
The Pharmacy Bridge provides a resource for access to medication for low-income, uninsured people who are patients of Coalition Clinics or Project Access. Individuals receive a voucher or pharmacy card which allows them to fill prescriptions at local pharmacies from a limited generic formulary.
This PBM manages the Coalition formulary at the pharmacy.
Because of the PBM:
- No off formulary medication will be filled
- No prescriptions for greater than three months will be filled
- No Prescriptions costing more than $150 will be filled
- No refills
- No narcotics
- Voucher expires after two weeks
